Before the Semester Starts

  1. Submit your ADVISING FORM to SZB216 to get you registered. Please ensure that ALL fields are completed (semester, unique #, site information, EEAA/PA approvals, etc.) Email the COMPLETED form to your Faculty Supervisor; name the PDF as follows: Last Name, Semester, Class, Unique (Example: Watson F19KIN627 11115)
  2. Where do you find if your site is an approved site?  Check here: Internships: Approved Sites
    1. If your site IS approved, fill in the Affiliate Program Agreement & Educational Experience Affiliation Agreement approval numbers on your advising form prior to turning in.
      If your site is NOT YET approved, you MUST share the links as explained on the Internship page above and your site must be in the process of getting approval when you register.  Include the date this was submitted on your advising form prior to turning in.
  3.  Submit the Student Fieldwork/Internship Agreement form via DocuSign
    1. Click the DocuSign link above.
    2. Enter your name and email address and then click "Begin Signing".
    3. Fill out the form, sign and initial, click "Finish".
    4. A copy of the completed form will be sent to you and your Faculty Supervisor.
    5. Download a copy of this form to save in your files.
  4. Submit the Fieldwork/Internship Supervision Contract form through DocuSign
    1. Click the DocuSign link above.
    2. Enter your name and email address, your Site Supervisor's name and email address, then click "Begin Signing".
    3. Select your Course and enter the Semester, then click "send".  (The Form will route to your Site Supervisor to complete)
    4. When you receive an email back from DocuSign, click "Review Document", read carefully and sign the form, click "Finish".
    5. A copy of the completed form will be sent to you, your Faculty Supervisor, and your Site Supervisor.
    6. Download a copy of this form to save in your files.

Mid-Term Evaluation Process

  1. Download a copy of the Mid-term Evaluation Form, enter your name, the course number and title, your Site Supervisor's name, and the Site, then email the evaluation form to your Site Supervisor.
  2. Have your Site Supervisor complete the form and review with you in person.
  3. To SUBMIT your completed Mid-Term Evaluation via DocuSign, follow these steps:
    1. Click this link: Initiate Mid-Term Evaluation Submission
    2. Enter your name and email address, your Site Supervisor's name and email address, then click "Begin Signing".
    3. Enter your Site Supervisor's name, evaluation date, and internship site, click "Send".
      1. Your Site Supervisor will receive an email to review the document (they will need the Stache Access Code in order to proceed; please be sure they know this and have the Access Code available before you click "Send" This Access Code will be given out via Canvas).
      2. After they enter the access code, they can fill out the document, attach the completed Mid-Term Evaluation form, and sign electronically (it will now route back to you).
    4. When you receive an email back from DocuSign, click "Review Document", read carefully and sign the form, click "Finish".
    5. A copy of the completed form will be sent to you, your Faculty Supervisor, and your Site Supervisor.
    6. Download a copy of this form to save in your files.

Final Evaluation Process

...

  1. Your Site Supervisor will receive an email to review the document (they will need the Stache Access Code in order to proceed; please be sure they know this and have the Access Code available before you click "Send" This Access Code will be given out via Canvas).
