  1. Login to the EM Customer Service Portal using the direct link above.

  2. Summary: Enter a title of the request like a subject in an email. 
  3. Event Name: Enter the full name of the event or function.
  4. Honorarium Company/Individual name: Enter the name of the company or individual.
  5. Honorarium Company/Individual email: Enter the email address of the business or individual.
  6. Honorarium Company/Individual phone number: Enter phone number for the company or individual.
  7. Honorarium Company/Individual address: Enter the address for the company or individual.
  8. Does this company/individual have an EID?: Select Yes, No, or Not sure
  9. Date of Contact: Select date from pop-up calendar.
  10. Date of event: Select date from pop-up calendar.
  11. Honorarium Amount: Enter the amount for honorarium. "$" sign not required.
  12. Travel Amount: Enter travel amount, if applicable.
  13. Comments/Additional information: Provide travel breakdown, date breakdowns, or other honorarium-related information.
  14. Attachment: Upload any documentation related to honorarium.
  15. Note: If not logged in, there will be an option to input your EID email at the bottom of the request.
  16. Click Send to submit request.
