1. Go to the Helpful Forms & Lists folder on the CLEE shared drive

2. Open the Forms - Accounting folder and then open the OOEF template, titled frm OOEF

3. Fill out the following fields located at the top of the form, as shown in Figure 1 below

a. Account to be charged: this should be a 10-digit account number
b. Form prepared by: enter your name or EID
c. Location/Place: where the food is being served
d. Date of Event
