CNS Flexible Work Arrangement (FWA) Request Proposal forms

- CNS FWA Request Proposal Form for 2 or fewer days remote weekly (supervisor approval required)
- CNS FWA Request Proposal Form for 3 or more days remote weekly (additional approvals required)
- CNS FWA Request Proposal Form for 100% remote work outside of Texas (dean's approval required)
