Individual instruction courses, such as X97 Research Problems, 698 Thesis, and X99 Dissertation require instructor approval before a student can sign up for the course.

 This process is hosted on DocuSign which can be found HERE.  Once you fill out the form, it will be sent to the faculty member. When they have reviewed and signed off, the form will be routed to the grad office, where we will sign you up for the course. If you are taking independent instruction courses with two different faculty members, complete a separate form for each faculty member.
