How To Submit Your Faculty and Staff Information Form

For new Faculty and Staff that are not yet in the Faculty and Staff directories.

Go to the Faculty Information Form page
Fill out the form as directed on the page

Note: All of the Ancillary information is optional. If you currently do not know the answers for certain responses, you can skip and submit the form regardless.
