Injury Assistance

In case of life-threatening emergency dial 911!

Note: Calling from a UT phone will make it easier for EMS to locate you.


The following applies when any employee of the University is injured while on the job. (Teaching assistants and GRAs are employees of the university.)

EMERGENCY

  1. If it is an emergency call 911 and ask to be transported to the nearest hospital.
  2. Forms may be submitted as soon as practicle after medical treatment has been provided.
    1. ‘First Report of Incident-Injury Memorandum’
    2. ‘Network Acknowledgement Form’


NON-EMERGENCY

  1. Notify your supervisor.
  2. Complete a ‘First Report of Incident-Injury Memorandum’ & ‘Network Acknowledgement Form’ (if the need for care is urgent, these papers can be completed within 48 hours of the incident).
  3. IF…
    1. The injury is minor and it is during normal business hours – you should visit the on-campus Healthpoint office in NOA 3.302 at 101 E 27th Street. They are open 8am-noon & 1pm-5pm on weekdays when the University is open.
    2. The injury is more serious or after hours (between 9am and 9pm every day) the closest provider is MedSpring at 2120 Guadalupe Street (right across from the HRC)
    3. The injury is more serious or after hours (24/7) the closest provider is St. David’s Occupational Health Services at 918 E. 32nd Street.
