Consent for Verbal Disclosure (UHS/CMHC)


A student can fill out a "Consent for Verbal Disclosure" from their UHS or CMHC web portal (MyUHS or MyCMHC). This is not the same as "The Authorization for Release of Medical Records" form which is specifically for requesting paper copies of medical records.  
The Consent for Verbal Disclosure of Health (or Mental Health) Information form is specifically for authorizing the verbal discussion of patient care with whomever the patient authorizes: parents, friends, outside providers, therapists, etc.   
If the student clicks on "Message" (1) and then "New Message" (2) they will get a list of secure messages that are available.

They will fill out a different form for CMHC vs UHS by clicking on the one they want, followed by the "Continue" button.

This is how the UHS form looks like (the CMHC form is a shorter version of this). The ** (red asterisks) mean it's a required field. The students may need help with some of the information.

If the student adds an attachment to the message, it is saved to "scanned documents" with no title, date, etc. associated with it.
