1) Why can't I complete the DocuSign process?

If you see the below picture, go to the email that you entered in the first step. There will be a email from DocuSign. Click on the secure link to complete the form.

2) I do not have a health permit needed to complete the application.

In order to apply, you must have the required documents. Below are sources that can help you find more information on obtaining a health permit.

• Outside of Austin: https://dshs.texas.gov/foodestablishments/permitting.aspx

3) I do not have a UT EID. 

In order to apply, you must have a UT EID. You may contact our office for assistance regarding this. Please email purchasing@austin.utexas.edu with "Catering Services - Vendor UT EID Request' in the subject line. There will be an additional process to get you started and it may delay the normal processing time for your application. 

4) I do not remember my UT EID. 

In order to apply, you must have a UT EID. You may contact our office for assistance regarding this. Please email purchasing@austin.utexas.edu with "Catering Services - Vendor UT EID Request' in the subject line. We will confirm your UT EID and get in contact with you.
