The individual must visit the ID Centerto have an EID Admin complete the merge.
Send an email to the individual with multiple EIDs - a follow-up phone call is recommended so that the individual has enough information to understand the process.
Include the ID Center address and a step-by-step instruction on how to initiate the process.
The employee must take valid proof of identity (Gov't issued ID, ID Card, etc)
Advise the employee to call you back on the main line (5-7943) when complete.
