Student Assessments
Field supervisors and cooperating teachers use formative assessments and summative assessments to give students feedback on their performance during PDS field experiences.
Preservice teachers use formative self-assessments to gauge their performance and align their goals with those of the field supervisor and cooperating teacher.
The Three-Person Conference provides an opportunity for them to collaborate, discuss the preservice teacher’s progress and any need for support in the placement, as well as set goals for the future.
Our field experience assessments are completed via Docusign PowerForms to allow users to complete and sign documents electronically and securely manage the student data.

FS Assessments are initiated by the Field Supervisor using a DocuSign PowerForm. In DocuSign, the Field Supervisor adds their first and last name and email & student teacher’s first and last name and UT email, then the assessment routes to the Field Supervisor to complete and sign. Next, it routes back to the student teacher to sign. Finally, the Faculty Area Coordinator signs the document. The document is now completed. All parties will receive an email to download the completed document, and it will automatically be uploaded to the UTeach Fine Arts BOX, which the Faculty Area Coordinator will add to the student teacher’s file. | CT Assessments are initiated by the Cooperating Teacher using a DocuSign PowerForm. Formative Assessments
Field supervisors and cooperating teachers use the Formative Assessments to evaluate student performance at the midterm of the Student Teacher semester. Check with your faculty area coordinator for specific due dates.
Summative Assessments
Field supervisors and cooperating teachers complete the student teachers' final PDS evaluation with the Summative Assessment at the end of the Student Teacher semester. Check with your faculty area coordinator for specific due dates.
