All forms need to be filled out through DocuSign. If there is a form that is not listed as a DocuSign, please email chemegrad@utexas.edu to inquire about the form you are needing.

You are required to submit completed DocuSign forms to the Graduate School! 

Envelope Errors

If you submitted a DocuSign envelope by mistake or need to make major adjustments, please void the envelope and create a new one. The Graduate Program Administrator can usually edit typos on forms, but not major adjustments. If you have questions on this, please email chemegrad@utexas.edu to inquire about your current envelope. Below are the steps on how to void an envelope:
