CARE Message/Referral Form
The CARE Message/Referral Form is a Qualtrics survey that allows for students to leave a message for a CARE Counselor and/or find out more information about the CARE program. The form also gives faculty/staff the opportunity to refer a student to the CMHC CARE program, as deemed appropriate. The aim of this survey is to provide mental health help and support to students who may be in need.
