  1. Conduct Image Quality Assurance (scanned records).  Paper copies of records should be destroyed only after the quality assurance review is conducted.
  2. Is the record confidential?  Yes:  If the content of the record is confidential or sensitive, shred the paper record.  No:  If the content is not confidential, tear the paper record in half and dispose of it in the paper recycling bin. If you are in doubt about the confidentiality or sensitivity of the record, shred it.
