What is the Written Picture Description?
The Written Picture Description is a classic assessment tool used in neuropsychology, particularly for evaluating written language production, aphasia, and speech disorders. Our team uses Cookie Theft picture from the Boston Diagnostic Aphasia Examination (BDAE) as the stimuli.
¿Qué es la Descripción escrita de una imagen?
La Descripción Escrita de una Imagen es una herramienta clásica de evaluación utilizada en neuropsicología, especialmente para valorar la producción del lenguaje escrito, la afasia y los trastornos del habla. Nuestro equipo utiliza la imagen del Robo de las Galletas del Examen Diagnóstico de Afasia de Boston (BDAE, por sus siglas en inglés) como estímulo.
When to Administer
The Written Picture Description task is administered:

Note/Nota:
The vast majority of our Catalan-Spanish bilingual participants have not received formal literacy instruction in Catalan. This means they may speak and understand Catalan fluently but have limited or no experience reading or writing in the language. This lack of formal education in Catalan introduces a significant confounding factor when comparing their performance to that of Spanish-dominant individuals who were likely educated exclusively in Spanish and are fully literate in it.
La gran mayoría de nuestros participantes bilingües catalán-español no ha recibido instrucción formal en lectoescritura en catalán. Esto significa que pueden hablar y comprender el catalán con fluidez, pero tienen una experiencia limitada o nula al leer o escribir en ese idioma. Esta falta de educación formal en catalán representa un factor de confusión importante al comparar su desempeño con el de personas dominantes en español, quienes probablemente fueron educadas exclusivamente en español y poseen un dominio completo de la lectoescritura en esa lengua.
Structure of the Test
The Written Picture Description test involves a picture that depicts a scene of a child trying to steal cookies from a jar while the mother is distracted. This task is used to assess how well a person can describe the scene and how they organize their thoughts and speech.
What would a Clinician observe:
The description is typically analyzed for completeness, clarity, grammatical structure, and relevant details.
Errors such as paraphasias (word substitutions), neologisms (made-up words), or agrammatism (simplified, incomplete sentences) are noted.
The overall quality of the description can help classify the severity and type of language impairment.
Estructura de la Prueba
La prueba de Descripción Escrita de una Imagen consiste en una escena que muestra a un niño intentando robar galletas de un frasco mientras la madre está distraída. Esta tarea se utiliza para evaluar qué tan bien una persona puede describir la escena y cómo organiza sus pensamientos y su lenguaje.
¿Qué observa el/la clínico/a?:
La descripción generalmente se analiza en cuanto a su grado de completitud, claridad, estructura gramatical y detalles relevantes.
Se toman nota de errores como parafasias (sustituciones de palabras), neologismos (palabras inventadas) o agramatismo (oraciones simplificadas o incompletas).
La calidad general de la descripción puede ayudar a clasificar la gravedad y el tipo de alteración del lenguaje.
How to Administer
Before starting, provide the patient with a sheet of paper and a pen/pencil OR , for remote administration, make sure the patient has a sheet of paper and a pen at home.
For remote administration, ensure the patient has a large enough sheet of paper to write comfortably and that they are using a pen. Ink is more clearly visible on Zoom than pencil lead.
Instruct the patient to “Write a description of everything you see in the picture. Try to write in complete sentences.”
Allow the participant to write at their own pace, this is not a timed task.
When the participant is ready take a picture or screenshot (if remote) of the patient’s response. This image must be uploaded to the RedCap instrument.
For remote administration, it is very important to take the screenshot during the session to incase there are any technical difficulties with the recording afterwards.
Finally, ask the participant to read their response out loud.
To-Do List
- Make sure to take a screenshot if the test is being collected remotely.
- Answer the questions on Redcap (i.e Was the written description of the image collected?)
- Upload the screenshot to RedCap and the patient’s folder
- Transcribe the patient’s written description in the text box provided.
Cómo Administrar
Antes de comenzar, proporcione al paciente una hoja de papel y un bolígrafo o lápiz. En caso de administración remota, asegúrese de que el paciente tenga una hoja de papel y un bolígrafo en casa.
Para la administración remota, verifique que la hoja sea lo suficientemente grande para que el paciente pueda escribir con comodidad y que esté utilizando bolígrafo. La tinta se ve con mayor claridad por Zoom que la mina del lápiz.
Indique al paciente:
"Escriba una descripción de todo lo que ve en la imagen. Trate de escribir en oraciones completas."Permita que el participante escriba a su propio ritmo; esta tarea no está cronometrada.
Cuando el participante termine, tome una fotografía o captura de pantalla (en caso de administración remota) de la respuesta escrita del paciente. Esta imagen debe subirse al instrumento correspondiente en RedCap.
En administración remota, es muy importante tomar la captura de pantalla durante la sesión, por si surgen dificultades técnicas con la grabación posteriormente.
Finalmente, pídale al participante que lea en voz alta lo que escribió.
Lista de tareas:
- Asegúrese de tomar una captura de pantalla si la prueba se está recolectando de forma remota.
- Conteste las preguntas en Redcap (por ejemplo, ¿Se recogió la descripción escrita de la imagen?)
- Suba la captura de pantalla a RedCap y a la carpeta del paciente.
- Transcribe la descripción escrita del paciente en el cuadro de texto proporcionado.
