What is the NIH Toolbox?

The NIH Toolbox tests are administered during the in-person session in the patient's dominant language by the clinician conducting the session. If the clinician is conducting the session remotely, the clinical assistant is responsible for administering the task in the patient's dominant language.

EXCEPTION:

If there is not enough time during the visit to administer the test, or it is decided that it is better to do it during another in-person session, any person trained in administering these tests may conduct the NIH Toolbox. However, it must still be administered in the patient's dominant language.

CREDENTIALS:

Both the username and password for the app are recorded in the corresponding section of the platform https://stache.utexas.edu/.

SOCIODEMOGRAPHIC DATA

To begin administering the test, some sociodemographic information about the patient must be entered, as shown in the video attached below. The necessary information about the patient and where to find it is detailed in the following table:

¿Qué es la pruebas de NIH Toolbox?

La pruebas de NIH Toolbox se administran en la sesión en persona del idioma dominante del paciente por parte del clínico que realiza la sesión. Si el clínico de la sesión está realizando la sesión en remoto, es la clínica asistente la que debe administrar la tarea en el idioma dominante del paciente.

EXCEPCIÓN:

Si durante la visita no se cuenta con el tiempo suficiente para administrar la prueba o se decide que es mejor hacerlo durante otra sesión en persona, podrá administrar el NIH Toolbox cualquier persona entrenada en la administración de estas pruebas pero tendrá que hacerlo en el idioma dominante del paciente.

Tanto el usuario como la contraseña de la aplicación están registrados en el apartado correspondiente de la plataforma https://stache.utexas.edu/.

DATOS SOCIODEMOGRÁFICOS

Para poder iniciar la administración de la prueba, debemos introducir algunos datos sociodemográficos del paciente al que se le va a administrar este test tal como figura en el vídeo adjuntado más adelante. La información necesaria sobre el paciente y dónde encontrarla se detalla en la siguiente tabla:

Structure of the Test

2. How to administer the assessment:
   a) Set the iPad volume to maximum.

   b) Do not interrupt the patient during the test. If there is any distraction, redirect them back to the task. It is important to tell the patient to place their hand near the iPad, ignoring the phrase "mire CASA."

   c) Inform the patient that we cannot provide feedback regarding the task.

1.2 Adaptations for Patients

1.2.1 With Dementia - Memory or Comprehension Issues

During the examples: if the patient does not fully understand the task and only presses the central image, the written instructions or the voice-over prompt should be repeated.

It should be emphasized that they use one hand to select an option. "Just press the one that is the same" should be directed if the patient perseveres too much with the task, but without providing feedback regarding the understanding of the task.

1.2.2 Adaptations for Patients with Executive Function Problems

Ensure that poor test performance is not due to a memory or comprehension issue. Any important observations made during test administration should be noted in the Redcap instrument.

Data

1. Once every two months, the assigned clinics at each center download the data from the iPad and send it to the account multilingual.aphasia.lab@gmail.com.

3. Once all the data has been sent to the MADRlab email, Estefanía García uploads the data to the NIH Toolbox Data Raw folder.

ANNUAL PAYMENT

The app payment is made annually on May 5th. The payment date is specified in Google Calendar, along with a reminder set for the previous week to allow for advance notice of the corresponding charge.

The payment methods available through the app are shown below:

IMPORTANT Currently, the data of Dr. Miguel Santos is added, so a reminder about the upcoming payment date should be made in order to decide whether to continue with the same payment method or, alternatively, consider the possibility of switching to one of the other two options available in the app.

2. Cómo administrar la prueba:

a) Poner el volumen del iPad al máximo.

b) No interrumpir al paciente durante la prueba. Si hay alguna distracción, habrá que redirigirlo hacía la tarea. Es importante decirle al paciente que coloque su mano cerca del Ipad ignorando la frase de "mire CASA".

c) Informar al paciente de que no podemos darle feedback respecto a la tarea.

1.2 Adaptaciones a pacientes

1.2.1 Con demencia - problemas memoria o comprensión

Durante los ejemplos: si el paciente no entiende bien la tarea y pulsa, únicamente, la imagen central, deben repetirse las instrucciones escritas o la consigna de la voz en off.  

Debe incidirse en que utilice una mano para que escoja una opción. "Sólo pulse la que es igual" se le orienta si persevera mucho con la tarea pero sin feedback respecto a la comprensión de la tarea. 

1.2.2 Adaptaciones con pacientes con problemas de función ejecutiva 

Asegurarse de que no hay un problema de memoria o comprensión detrás del mal rendimiento de la prueba. Debe tomarse nota en el instrumento de Redcap sobre cualquier aspecto importante que se haya apreciado durante la administración de la prueba. 

Datos

1. Una vez cada 2 meses las clínicas asignadas de cada centro descargan los datos del Ipad y los envían a la cuenta multilingual.aphasia.lab@gmail.com.

3. Una vez todos los datos estén enviados al correo MADRlab, Estefanía García sube los datos a carpeta de NIH Toolbox de Data Raw.
   
   ⛑️ Si en algún momento las IPAD no permite descargar la data, sigue los siguientes pasos:
   PASO 1: Verificar cuenta de icloud: Stephanie Grasso.
   PASO 2: Revisa que la versión instalada sea version 1.28.7240
   PASO 3: Y sigue todos los pasos de descarga tradicional.

PAGO ANUAL

El pago de la aplicación se realiza de forma anual el día 5 de mayo. En Google Calendar se especifica la fecha de pago y un recordatorio la semana anterior para poder anticipar el cobro del importe correspondiente.

Las formas de pago que permite la aplicación son las que se muestran a continuación:

IMPORTANTE: Actualmente están añadidos los datos del Dr. Miguel Santos, por lo que debe realizarse un aviso de la proximidad de la fecha de pago para poder decidir si continuar con el mismo método de cobro o si, por el contrario, se valora la posibilidad de hacer un cambio a cualquiera de las otras dos opciones disponibles por el aplicativo.

La ruta para aplicar la tarea y hacer el naming convention es igual a lo descrito en este SOP.
Es importante, vigilar que NO se desvincule el email de la cuenta de APPLE. Con el fin de descargar correctamente la data.
